New Appointment Referrals
For a new appointment referral to one of our physicians and to obtain information about our clinic locations:

Preparing for Your Appointment
Please arrive about 15 minutes before your scheduled appointment time to check in at Registration desk, and then go to clinic reception desk. Please bring the following items to your appointment:

Printed and completed forms in Patient Packet for Orthopaedics or Patient Packet for Orthopaedics Oncology if you are new to our clinics.
A referral from your Primary Care Provider. Please check with your insurance company to find out if you need to obtain a referral before you can see a specialist physician.
X-rays, notes, reports or any other documentation regarding your condition from the Emergency Department or a referring physician.
Insurance card, photo ID, and co-payment
